Yes, we are currently offering tours by appointment for those interested in membership. Schedule your tour online. (Please note that not all areas can be accessed at this time. Once a physician reaches capacity, you can put your name on a waitlist. As openings become available, we will contact those on the list (in order). Additional providers and locations may be added in the future, based on demand.
This is an annual membership. You can choose to be billed monthly at $85 per month or save 10% by paying your annual membership fee up front at $918 per year.

No, the annual membership fee is not covered by insurance. However, traditional primary care services such as office visits and lab tests will be billed to your insurance as usual.
The membership does not cover clinical medical services. Office visits, lab work, imaging, prescriptions, specialist visits, and all other medically necessary services are billed to your health insurance separately. Copays, deductibles, and coinsurance may apply depending on your plan.
Lifestyle Medicine Primary Care membership fees are not covered by Medicare, Medicaid or other insurance. However, the program accepts major insurance plans for office visits and other clinical services. See a list of contracted plans.
Yes. Copays, deductibles, and coinsurance apply to clinical services as determined by your health insurance plan. The membership fee is separate and does not affect how your insurance processes claims for clinical services.
